Lead agencies must use the Residential Services Tool RS Tool for service planning and rate setting for adult foster care and customized living services. The longstanding Excel-based workbook was retired. DHS will not publish new versions of the workbook. DHS will continue to provide technical support for existing Excel workbooks as needed. All customized living, adult foster care, and residential care providers must enroll in Minnesota Health Care Programs.

County or tribal lead agency staff act as agents for the state Medicaid agency DHS and are responsible to administer programs. At the time of a survey, investigation, or other licensing activity, the licensee must provide the department access to or copy of the uniform assessment tool as required under Minnesota Statutes, section G.
